Normal localization for papilla of Vateri are commonly on the posterior wall in the second portion of duodenum, and choledochus lies down adjacent to the medial wall of the second portion of the duodenum, and opens into duodenum via papilla of Vateri. Choledochus can have proximal (stomach, pylorus and duodenal bulb) or distal (third or fourth portion of duodenum) ectopic openings beside this normal localisation. Ectopic opening of the biliary system into the stomach is quite rare. The most common site for gastric ectopic opening is the corpus and more rarely the antrum; in most cases there are double common bile ducts. Here we report a case with apical stenosis, ectopic opening of choledochus to duodenal bulb, and choledochogastric fistulae.
