“Walled-off” pankreatik nekroz siddetli akut pankreatitin yasami tehdit eden komplikasyonlarindan biridir. Enfekte ve/veya semptomatik nekroz varliginda girisim gerektirmektedir. Geleneksel tedavi yaklasimi olan açik nekrozektomi yöntemi yüksek morbidite ve mortalite riski tasi-maktadir. Endoskopik nekrozektomi son yillarda yapilan ve açik cerrahi nekrozektomiye kiyasla daha az morbidite ve mortaliteye sahip bir yöntemdir. Endoskopik drenajda genellikle transgastrik veya transduodenal trakt kullanilmaktadir. Önceki çalismalarda transpapiller yaklasimin tedavi için yeterli olmadigi bildirilmistir. Bu yazida akut nekrotizan pankreatit sonrasi pankreas kuyruk kesiminde Walled-off pancreatik nekroz gelisen hastada pankreas kanali araciligi ile endoskopik drenaji saglanan bir olguyu sunuyoruz.
Walled-off pancreatic necrosis is one of the life-threatening complications seen in acute pancreatitis. Walled-off pancreatic necrosis requires intervention in the presence of infectious and/or symptomatic necrosis. Surgical necrosectomy, the conventional treatment approach, has a high risk of morbidity and mortality. Endoscopic necrosectomy is a recently applied method that has fewer morbidity and mortality rates than the open surgical method. The transgastric or transduodenal tract is usually used in the endoscopic drainage. Previous reports in the literature show that a transpapillary approach is insufficient for treatment. In this report, we present a patient who developed walled-off pancreatic necrosis in the pancreatic tail segment after necrotizing pancreatitis and underwent endoscopic drainage through the pancreatic duct.
