Tiroid karsinomu tüm kanserler içinde %1 oraninda görülürken medüller tiroid karsinomu, tiroid kanserlerinin yaklasik %5-10’unu olusturur. Sporadik ya da ailevi olabilir. Özellikle sporadik formlar daha geç tani almaktadir. Klinik semptomlar genellikle lokal invazyon ve hormon sekresyonuna bagli ciltte kizarma ve diyare gibi semptomlardir. Ekstraintestinal bulgular ülseratif kolitin iyi bilinen klinik özelliklerindendir. Bunlar tani öncesi veya klinik seyir sirasinda ortaya çikabilir. Literatürde medüller tiroid karsinomu ile ülseratif kolit birlikteligi simdiye kadar gösterilmemis olup sadece papiller tiroid kanseri ile eslik eden bir olgu sunumu vardir. Ayrica medüller tiroid karsinomasi'nin ishal sikayeti ile beraber gitmesi, ülseratif kolit klinigi nedeni ile hastaya tanisal yaklasimi zorlastirmaktadir. Ülseratif kolit tanisi mevcut hastalarimizda her ishal atagini mevcut taniya baglamayip diger nedenler de dislanmalidir. Bu yazida ülseratif kolit seyrinde gelisen bir medüller karsinomu vakasini sunduk.
Thyroid carcinoma accounts for 1% of all malignancies, and medullary thyroid carcinoma accounts for 5-10% of the thyroid carcinomas. It can be sporadic or familial. The sporadic forms are especially diagnosed late in the course of the disease. Thyroid carcinoma usually presents with symptoms and signs related to local invasion to adjacent structures and hormone secretion, such as flushing and diarrhea. Extraintestinal manifestations are well-known clinical features of ulcerative colitis. They develop prior to or during the course of the disease. Medullary thyroid carcinoma has not been reported in patients with ulcerative colitis to date, and only one case of papillary thyroid carcinoma has been reported. Because medullary thyroid carcinoma can present with diarrhea, it is difficult to make a diagnosis in patients with ulcerative colitis, which also manifests with diarrhea. Therefore, all diarrheal attacks should not be attributed to ulcerative colitis in these patients; instead, other possibilities should be evaluated. We report a case of medullary thyroid carcinoma developing in the course of ulcerative colitis.
