Giris ve Amaç: Post endoskopik retrograd kolanjiografi pankreatitini önlemek amaciyla pankreas kanalina profilaktik stent koymak riskli kisilerde pankreatit gelisimini önemli ölçüde azaltmaktadir. Profilaktik olarak yerlestirilen stentlerin bir süre sonra spontan olarak düsmesi istenilir, spontan olarak düsmeyen stentler pankreatik duktal hasara ve stentlerle iliskili diger komplikasyonlara yol açabileceginden endoskopik olarak çikartilmalidirlar. Bu çalismada profilaktik amaçla 5 Fr plastik, pankreatik stent yerlestirilen hastalarda stentlerin 3 günün sonundaki spontan düsme orani ve bu hasta grubundaki pankreatit sikligini belirlemeyi amaçladik. Gereç ve Yöntem:Mayis 2010 ve Eylül 2012 tarihleri arasinda post endoskopik retrograd kolanjiografi pankreatiti profilaksisi amaciyla pankreatik stent konulan 80 hastanin bilgileri incelendi. Bu hastalardan 5 Fr pankreatik stent konulan 67 hasta çalismaya alindi. Bulgular: Çalismaya alinan 67 hastanin 33’ünde (%49.3) 3 günün sonunda pankreatik stentler spontan olarak düstü, proflaktik amaçla 5 Fr pankreatik stent yerlestirilen hasta grubundaki sadece 2 hastada (%3) hafif pankreatit görüldü. Sonuç:Profilaktik amaçla konulan 5 Fr plastik pankreatik stentlerin yarisi 3 günün sonunda spontan olarak migre olmaktadir. Yüksek riskli hastalarda profilaktik amaçla konulan bu stentler pankreatit gelisimi riskini önemli ölçüde azaltirlar.
Background and Aims:Pancreatic plastic stent placement to prevent post-endoscopic retrograde cholangiopancreatography pancreatitis significantly reduces the frequency of pancreatitis in patients deemed to be high risk. Spontaneous migration of these stents is desirable and endoscopic removal is advised for stents that do not migrate due to the increased risk they pose for ductal injury and other stent related complications. In this study, we looked at the frequency of spontaneous migration of plastic stents within 3 days following placement and the occurence of post endoscopic retrograde cholangiopancreatography pancreatitis in these patients. Materials and Methods:The medical records of 80 patients who received pancreatic prophylactic stents between May 2010 and September 2012 were reviewed; the records of 67 patients with 5 Fr stents were studied. Results:In 33 (49.3 %) of the 67 patients in the study group, stents spontaneously migrated to the intestinal lumen by the end of 3 days. Mild pancreatitis occured in only 2 patients (3%) with prophylactic pancreatic plastic stents. Conclusion:Almost half of 5Fr pancreatic plastic stents placed to prevent post-endoscopic retrograde cholangiopancreatography pancreatitis migrated by the end of 3 days. The use of these stents significantly reduced the frequency of post-endoscopic retrograde cholangiopancreatography pancreatitis in high risk patients
